Family Hospice & Palliative Care is a locally owned and operated non-profit organization whose mission is to bring hope, comfort and dignity along life’s final journey. Our professional team consisting of nurses, social workers, chaplains, dietitians, massage therapists, hospice aides, bereavement companions and volunteers care for a daily average of 115 patients and their loved ones in northeast Indiana in the following counties: Adams, Allen, Blackford, Huntington, Jay and Wells.Family Hospice & Palliative Care is committed to providing you and your loved ones quality, compassionate care at the end of life. Our team of professionals is available 24 hours a day to work alongside you and your family members offering personalized medical, emotional and spiritual support wherever you are….nursing facility, assisted living, hospital or your home.
Our organization is the only hospice provider in Indiana that has two Board Certified Hospice & Palliative Medicine physicians and a pediatrician on staff who keep us on the cutting edge of pain and symptom management. For those who are not terminally ill but have a serious illness with complicated pain and/or symptom management needs, Family Hospice & Palliative Care created a special program, Comfort-Focused Palliative Care, to assist your family physician in meeting your individualized needs. - If I choose hospice care, am I giving up hope?
- When you choose hospice care, you are redefining hope. Our goal is to help you focus on making the most out of life, which may come in many forms…. hope of a final trip to see distant relatives, hope to be pain free, hope to spend as much time as possible with loved ones. The focus of your hospice team is to assist you with identifying your goals, fulfilling your wishes and maintaining hope throughout this journey.
- What diagnoses are covered under hospice care?
- Hospice Care was created to provide assistance to anyone during their end of life journey regardless of their diagnosis. In 2008, 73% of the Family Hospice & Palliative Care patients had a non-cancer diagnosis. Some other common diagnoses in hospice are heart disease, dementia, kidney disease, lung disease, liver disease and other conditions.
- If I choose hospice services, can I remain with my primary physician?
- No one knows you better than your own physician. This is why Family Hospice & Palliative Care encourages you to remain with your own physician who then becomes a part of our hospice team. Our hospice physicians will work closely with your physician to develop a plan of care that incorporates your personal goals.
- When should I contact Family Hospice? Isn’t hospice care for individuals near death?
- Receiving hospice care does not mean you are near death. It means you have chosen a different level of care that focuses on comfort versus cure. Our hospice team provides unique physical, emotional and spiritual care. The sooner our hospice team becomes involved with you and your loved ones, the sooner we can work together and ensure your comfort. One commonality that we find amongst our patients is that once they have chosen hospice services, their quality of life improves. - How is hospice care paid for?
- Hospice services are a free benefit for Medicare and Medicaid recipients. Family Hospice & Palliative Care also accepts payment from Commercial insurance companies for which there would be little or no charge to the insured. The hospice benefit covers costs related to the life-limiting illness, including medications, medical equipment, medical supplies and the valuable time and expertise of our team of professionals. A financial assessment can be completed for persons not covered under insurance.
