Family Hospice & Palliative Care is a locally owned and operated non-profit organization whose mission is to bring hope, comfort and dignity along life’s final journey. Our professional team consisting of nurses, social workers, chaplains, dietitians, massage therapists, hospice aides, bereavement companions and volunteers care for a daily average of 115 patients and their loved ones in northeast Indiana in the following counties: Adams, Allen, Blackford, Huntington, Jay and Wells.

Our organization is the only hospice provider in Indiana that has two Board Certified Hospice & Palliative Medicine physicians and a pediatrician on staff who keep us on the cutting edge of pain and symptom management.
