INDIANAPOLIS (News Release) - Earlier today, the Indiana Department of Administration (IDOA) sent notification letters to 8,775 current and former state employees whose Social Security Numbers were inadvertently included in a contract solicitation file posted to the IDOA Procurement website.
The file, which was posted for approximately two hours on Tuesday, Jan. 20, was immediately removed from the site upon discovery by an IDOA employee, and IDOA immediately involved the Indiana Office of Technology’s Information Security Incident Response Team (ISIRT). The team determined that four (4) entities accessed the file from the site. IDOA has secured sworn affidavits (or the equivalent) from three of the companies affirming that they have destroyed the information and that prior to destruction, they did not forward, print, copy, or otherwise maintain or distribute the downloaded file containing employee Social Security Numbers. IDOA has subpoenaed an internet service provider for the name of the fourth entity who accessed the file and will seek a similar documentation that the file has since been destroyed.
As required by law, IDOA notified the Attorney General's Office about the inadvertent release.
“We are doing everything we can to minimize any risk associated with this incident. We don’t take this lightly. We are reviewing and strengthening our procedures to ensure that this does not happen again,” said newly-appointed IDOA Commissioner Mark W. Everson.
Since Jan. 1, 2004, IDOA’s Procurement Division has released more than 1450 procurements of over $75,000, including more than 528 RFPs, without incident.
