FORT WAYNE, Ind. (21Alive) - A Fort Wayne man died Monday afternoon after being pulled from a lake in a southwest housing addition.

Witnesses tell the Indiana Department of Natural Resources that around 2:30, Adam Palm, 31, was in the lake at Kekionga Shores, when he swam to retrieve a beach ball while playing with his children.

Palm began to struggle, went under water and never resurfaced, conservation officers say.

The Fort Wayne Water Rescue Team arrived at the lake, and found Palm in 10 feet of water, 30 feet from shore.

Palm was flown to a hospital, where he later died.
